Unloading cecostomium at appendicular peritonitis in children

Nnotation: The study of purulent peritonitis is one of the important problems of surgery. The search for new methods to combat intra-abdominal infection and intestinal paresis does not stop. Total abscessed peritonitis is the most severe form of appendicular peritonitis, which is characterized by the formation in the abdominal cavity of multiple sensitized abscesses, the development of sepsis syndromes and multiple organ failure, and often the transition to an infectious toxic shock by the time patients arrive at the hospital. The main objectives of the surgical intervention, in addition to appendectomy, for total abscessed peritonitis, is the rehabilitation of all the concocted spaces, total resection of the omentum, decompression of the intestine, and dynamic monitoring of the course of the inflammatory process in the abdominal cavity by creating a laparostomy according to indications. In conditions of total abscessed peritonitis with the development of total visceritis, the use of parietal cecostomy allows the most rapid and atraumatic intubation of the small intestine through the Baugin gate. Since 2009, cecostomy has been put into practice with total appendicular peritonitis. The use of cecostomy for total abscessed peritonitis of appendicular origin, according to our observations, has shown its advantages over enterostomy.
