Design and analysis of application of minimal extracorporeal circuit to perform hyperthermic intraperitoneal chemoperfusion in experimental settings

Hyperthermic intraperitoneal chemoperfusion (HIPEC) is a high-tech modern treatment modality for patients with peritoneal carcinomatosis from the tumors affecting the peritoneum. Technical challenges associated with this procedure cause the search and introduction of new concepts of HIPEC. Minimal extracorporeal circuit (mec) is one of them. The aim of the study was to develop minimal extracorporeal circuit to perform HIPEC and to conduct comparative analysis of its efficacy in a rat model of peritoneal carcinomatosis from ovarian cancer. The study was carried out in 32 female outbred Wistar rats with ascetic ovarian cancer which was inoculated intraperitoneally at a certain amount of ovarian cancer cells. Following inoculation rats were randomized into two groups: group I - control, HIPEC with cisplatin (20 mg/kg body weight) performed according to the previously created thechnique (n-17), group II - HIPEC with cisplatin (20 mg/kg body weight) using mec and laser transmitter (n=15). In both groups HIPEC was performed in 48 hours after ovarian cancer inoculation. Experimental perfusion model of HIPEC incorporated mec was developed and successfully tested and its main advantages such as less number of postoperative complications and improved survival of rats with advanced ovarian cancer compared to HIPEC without mec were demonstrated in the study.
