Issues of detection, record and registration of suicidal behavior are discussed. Major complexities and basic organizational principles of work in this direction are noted. The organizational model of the suicidological register is substantiated. As an example, the work experience of the suicidal service in Tyumen (Western Siberia) is given. It is shown that the opening of the Suicidological Register in the region helped to increase the effectiveness of identifying and recording suicidal attempts. One of the criteria of the work of the register is the ratio of the number of completed suicides and attempts. In the first year of the study (2012), the number of attempts exceeded the number of lethal cases by 5.6 times. In the following years, along with the system's improvement, there is a tendency to widen the gap, which was maximum marked in 2014 (1: 15.2) and 2017 (1: 13.7). On average, in 6 years (2012-2017 гг..) this ratio has become to equal 1: 8.5. An increase in the number of cases of suicide attempts contributed to an increase in the number of people receiving the necessary volume of psycho-correctional care. This was a factor in reducing suicide mortality in Tyumen by 3.8 times - from 25.2 in 2010 to 6.6 in 2017 (per 100,000 population). In conclusion, the authors conclude that the proposed recording model is sufficiently effective and offer its wider use in the work of regional suicidal services. The authors also develop and propose a model of the federal suicidological register.
