Radical nephrectomy and partial nephrectomy provide equivalent oncological outcomes. To prevent the progression of chronic kidney disease the significance of nephron-sparing surgery increases. Partial nephrectomy is routinely performed under total ischemia with a renal vessels being clamped. Every minute of warm ischemia contributes for the acute renal failure and chronic kidney disease. To improve functional outcomes of nephron-sparing surgery it was suggested to perform the procedure under local ischemia. First positive results show the effectiveness of the new method and help to do further steps to improve it.
