Treatment results and consequences of leg injury

The aim of this study was analysis the long-term results of treatment of severe injuries and gunshot wounds to the lower leg, which combine defects bone and soft tissues of the lower leg, and analyze them from the point of view of the consequences of the severe course of the wound process.Material and method. A retrospective analysis was carried out of 53 patients treated for gunshot and severe open wounds of the lower leg with extensive soft tissue wounds and a segmental tibia bone defect of more than 5 cm.Results. All patients managed to eliminate soft tissue defects and replace the defect in the tibia at various times after the operation; in a comparative analysis of the time of consolidation, the mean values were 163.2 ± 8.7 days in group I, and 189.6 ± 15.3 days in group II, and in group III - 154.9 ± 7.1 days. Complete restoration of range of motion was observed only in 5 (9.4%) patients, 13 (24.5%) wounded had minor joint dysfunctions, 20 (37.7%) had moderate dysfunctions, 15 (28.3%) had signifi ant dysfunctions. ) wounded. Post-traumatic contractures caused fractures of the shin bones in the long-term period (more than 10 years) in 10 patients, which amounted to 18.9%. Treatment was carried out with a rod extra focal apparatus, without intervention in the fracture area, the consolidation time did not exceed the usual fusion time.Conclusion: the treatment of pathological fractures in the regenerate area has features, but with a full restoration of the skin and muscle cuff, the fusion of such fractures can proceed favorably.
