Results of operation of «Left-atrial labyrinth» in patients with mitral valve pathology

We examined 100 patients (mean age 62.7 ± 4.5) with MV pathology: 81 patients with long persistent atrial fibrillation (AF) (duration 18 ± 5.1 months), 15 patients with persistent AF, and 4 patients with paroxysmal AF. All patients were operated on for combined pathology of the MV in the period from 2010 to 2012. Etiology of MV damage: 62 patients had degenerative MV damage, 30 patients had ischemic mitral insufficiency, 4 patients had heart disease RBS, and another 4 patients had idiopathic annulectasia and MV insufficiency. In the pre-and postoperative period, we took the patients' clinical data, as well as their echo CG data, Holter monitoring, and coronary angiography. All the patients had maze 3 procedure with the correction of the pathology of the mitral valve. 31 patients had this procedure performed in combination with coronary artery bypass grafting, 33 patients underwent correction of tricuspid insufficiency. Hospital mortality was 3%: 3 patients died from multiple organ failure. During the first month after the operation, 85 (89%) patients had sinus rhythm (n = 96); of these 18% had the mechanical systole of the left atrium (LA). 8 (8.2%) patients had atrial fibrillation, while 3(2.8%) patients had to have an electric cardiac pacemaker installed. 6 months after, 59(83%) patients of the post-op group (n=71) had sinus rhythm, of these 55% had a mechanical systole of the LA. One year postop, 31 (81%) patients had sinus rhythm, with a mechanical systole of the LA in 48% of cases. During the year, another 4 patients needed to have a dual-chamber electric pacemaker installed for atrioventricular block. Freedom from atrial fibrillation over the 12-month period was 80%. Freedom from acute cerebrovascular disorders was 98.7%.
