Results of application of differentiated tactics of surgical treatment of patients with various forms of chronic pancreatitis

Article is devoted to a problem of surgical treatment of chronic pancreatitis. Diagnostic criteria are developed and four clinical and anatomical forms of chronic pancreatitis are allocated: with the isolated defeat of the head, primary defeat of a head, with defeat of distal sections of the pancreas, pancreatitis without focal lesions of the pancreas. Patients are divided into 2 groups: main (n = 152) and comparison (n = 163). In the main group original methods of a longitudinal pancreaticojejunostomy with a resection of a forward surface of a pancreas (n = 6), pancreaticogastrostomy, which is consisted in coupling similar peritonizations by wall of the stomach (n = 7), a bilateral pancreaticojejunostomy (n = 4). Postoperative lethality is lowered from 4,3% to 1,4%, and number of repeated operations for complications from 13,5% to 9,9%.
