Results of local infiltration anesthesia in total arthroplasty of the knee

We conducted a single-center prospective study of 159 patients randomized into three groups who underwent primary total knee arthroplasty (TKA) under spinal anesthesia.The aim of the work was to evaluate the effectiveness of the use of local infiltration anesthesia in primary TKA.Methods: In the first group, periarticular local infiltration anesthesia (MIA) was performed, in the second group, blockade of the popliteal artery (BPA) region was performed, and the third group was the control group, in which additional anesthesia was not performed in the postoperative period. Comparisons were made using the scales VAS, KSS, strength of the quadriceps femoris muscle.Result. The study showed a similar level of pain in MIA (group 1) and periarterial blockade of the popliteal artery (group 2), but the best dynamics of recoveryof quadriceps muscle strength (straight leg lift) during the first six postoperative days was shown by local infiltration anesthesia. Lower limb function according to the KKS scale significantly improved in the groups with no statistically significant difference before surgery, at six and twelve months.Conclusion. The use of MIA makes it possible to achieve adequate pain relief in the early postoperative period, which allows the initiation of early activerehabilitation and good functional results of the primary TKA.
