The article presents a clinical case of the development of a massive paradoxical embolism during the removal of a large vestibular schwannoma. The clinical case demonstrates one of the complications that occur during neurosurgical operations performed in the patient’s sitting positionIn this clinical case, against the background of an air embolism of cerebral vessels, multiple foci of cerebral ischemia formed, which are of interest mainly due to the cortical localization of the formed multiple foci, which determined the rapid regression of gross postoperative neurological deficit.
