Trauma is the leading cause of death in people under 50 years of age. The combined injury is characterized by the simultaneous damage of sev-eral anatomical and functional areas and is observed in 50-70% of victims with severe mechanical damage. Mortality in combined trauma reaches 85%, in case of cranioabdominal injury it is about 76.2%. Severe combined head and abdominal trauma is one of the most difficult and unresolved problems of medicine and has great social significance, both because of its prevalence and a large percentage of unsatisfactory treatment outcomes.The main of this review is to familiarize surgeons with up-to-date data, which, from the standpoint of evidence-based medicine, justify the need to change traditional surgical dogmas and tactical principles to improve the results of treatment of patients with combined injuries of the abdominal organs. To solve the problem it is advisable: the concentration of victims in trauma centers; introduction into practice of a single protocol for diagnosis and treat-ment, taking into account an objective assessment of the severity of injuries; ensuring round-the-clock availability of high-tech diagnostic and treatment methods (MSCT, angiography, endovideosurgery); The introduction of the phased treatment of abdominal trauma in trauma centers level II and III.
