In surgical treatment of 67 patients with obstructive jaundice, liver tissue taken during surgery, bile duct and blood serum in the comparative assessment of the dynamics of studied concentration TNFα and IL-6. All patients postoperatively received conventional basic intensive therapy. The degree of hepatic dysfunction was assessed using markers of cholestasis (total bilirubin level and the activity of alkaline phosphatase) and hepatocyte cytolysis (alanine aminotransferase and aspartate aminotransferase). Our research has shown that the depth of the changes studied parameters depends on the severity of hepatic dysfunction. TNFα content was higher in the bile duct, then in the blood and liver tissue. For IL-6 is a characteristic higher concentration in the bile and then into the liver and blood. The simultaneous increase in the content of cytokines in obstructive jaundice in the liver, bile and blood indicates the development of cytokine imbalance at local and systemic level. In the postoperative period revealed the same type of directional changes in the level TNFα and IL-6 in the bile and the blood reflects the peculiarities of the pathological process.
