Objective: To improve the results of treatment of patients suffering from rectocele combined with chronic internal hemorrhoids by applying circular endorectal resection of muco-submucosal layer of the distal colon. Materials and Methods: The results of surgical treatment of 356 patients suffering from chronic rectocele and internal hemorrhoids treated in coloproctology department from 2006 to 2015 were analyzed. The patients were divided into 2 groups: I group - 165 patients suffering rectocele with chronic internal hemorrhoids of stage II-IV after Longo operation, II group - 191 women with chronic internal hemorrhoids of stage II-IV after a closed hemorrhoidectomy. Results: circular resection of muco-submucosal layer of the rectum in patients with rectocele and chronic hemorrhoids is pathogenetically justified and effective treatment. This method allows to eliminate the excess of mucosal prolapse, and at the same time to create a strong connective muscular frame of the intestinal wall. The Longo operation as a method of treatment for patients with a combination of rectocele and chronic internal hemorrhoids allowed to achieve good and satisfactory results in 97.9% of cases, which makes it to be considered a procedure of choice. Conclusion: The advantages of this method are the low invasiveness of surgical benefit, absence of expressed pain syndrome in the postoperative period, shortening of hospital stay and time of medical and social rehabilitation of patients.
