Comparison of in-hospital and long-term results of mitral valve repair surgery in infective endocarditis and other types of pathology

Introduction. The method of choice for correction of mitral regurgitation is mitral valve repair. Due to the different nature of these diseases and the details of surgical approaches, the results of valve sparing operations in different nosologies may vary. Purpose. Comparison of in-hospital and long-term results of mitral valve repair surgery in infective endocarditis, fibroelastic deficiency, Barlow’s disease and ischemic mitral regurgitation. Material and methods. In-hospital and 3-year long-term clinical and echocardiographic results of 108 patients with infective endocarditis (22), fibroelastic deficiency (36), Barlow’s disease (11) and ischemic mitral regurgitation (39 patients) were studied. All patients have undergone mitral valve repair surgery. Results. There were no cases of in-hospital deaths in any group. Postoperatively in-hospital nonlethal complications were observed in 4 (18.2%) patients from infective endocarditis group, in 3 (8.3%) patients from fibroelastic deficiency group, in 1 (9.0%) patients from Barlow’s disease group and in 4 (10.2%) patients from ischemic mitral regurgitation group. In-hospital reduce of the left ventricle size was found in all groups of patients. In patients with IE this index has shown the lowest deviation from the normal size. 3-years actuarial survival was by groups: I - 86.4%, II - 91.0%, III - 100%, IV - 91.9%. 3-year follow-up has revealed good results in 88.8% of all patients after mitral valve repair. Good long-term results in this period in groups: Group 1 - 95.5%; Group 2 - 91.7%; Group 3 - 72.7%; Group 4 - 86.8%.The best echocardiographic long-term results were found in patients with infective endocarditis. Conclusion. Mitral valve repair method allows achieving good results of treatment in patients with infective endocarditis, fibroelastic deficiency, Barlow’s disease and ischemic mitral regurgitation, but the number of favorable outcomes in these groups is different.
