Comparative assessment of results of pylorus-preserving and conventional pancreatoduodenal resections

This review presents the analysis of results of 85 pancreatoduodenal resections performed during the period from 1995 to 2004. Out of them, there were 73 conventional gastropancreatoduodenal resections and 12 pylorus-and stomach-preserving resections. When pylorus-conserving resection took place, pancreatoanastomosis - duodeno-duadenoanastomosis - hepaticoenteroanastomosis according to Roux were performed. At this stage of surgery the natural food passage via the stomach, the pylorus and the part of duodenum was maintained preventing the development of post-gastroresection syndromes and resulting in favorable postoperative period. Reduction in the resection extension due to preservation of pylorus, stomach and the part of duodenum had no influence on the tumor recurrence and the overall survival of patients. Our findings are in agreement with reported data concerning the fact that pyloric and gastric preservation during pancreatoduodenal resection does not reduce the radicality of surgery.
