Prostate cancer staging aft er radical prostatectomy

Prostate gland malignant tumors – actual problem of modern oncology. Radical prostatectomy (RPE) is a most effi cient treatment option with patients’ high survival and quality of life rates. Moreover RPE allows precise morphological staging, which has a direct relation for prognosis and further treatment options. For prostate cancer epidemiological evaluation in Krasnodar territory we used clinical statistic data from local cancer-register for 1999–2009 period. A growth of prostate cancer incidence, number of patients registered and mortality were estimated. RPE was introduced in local oncological department since 2004. In 2004 – 2009 years 316 RPE were performed. 302 cases with reliable data were analyzed. For selection of patients with localized form of disease 3 level examination algorithm with prostate biopsy, CT and MRI was used. Aft er RPE in 256 cases stage did not changed, in 46 cases prostate cancer stage increased. Th ese results were compared with Partin staging tables. It was mentioned lower mistake rate of presurgical staging than with the help of tables. Infl uence of PSA level and Gleason score on cancer staging was analyzed. It was revealed that postsurgical stage increase possibility is reliably higher with Gleason score ≥5. Negative infl uence is mentioned in PSA ≥ 15 ng/ml. Stage increase has a worse prognosis and oft en requires a systemic therapy.
