Structural and functional changes of the heart of patients of senior and old age with myocardial infarction with low ejection fraction in different periods of disease

This paper presents the structural and functional changes of the heart in elderly patients with myocardial infarction transfer initially low ejection fraction in the acute period in various periods of myocardial infarction (MI) and postinfarction cardiosclerosis. The aim of the study - to evaluate morfologikal changes and changes of cardiac function in elderly patients with myocardial infarction with low baseline ejection fraction (EF) and follow the dynamics of these indicators for extended surveillance. In observation included 135 patients with myocardial infarction, selected on the basis of the presence of low EF in the first 48 hours by echocardiography. All patients echocardiography study was done in the first 48 hours, 21 days (5 days clearance), 12 months, 24 months, 36 months, 60 months. Patients older than 60 years were identified in the elderly group, which has been the subject of study, a person younger than 60 years were in the control group. On the basis of the data obtained that the elderly base lower EF and more reduced systolic function, more often observed dilatation of the left cavities, more wall thickness of the left ventricle (LV), is more common anterior transmural myocardial localization involved with lots of affected segments. Systolic function in older patients (survivors) is slowly improving to five years of follow. Diastolic function was broken almost the same in both groups, more elderly deteriorated more slowly than younger people. Accordingly, the conclusions were made about the need to not less aggressive therapy aimed at improving systolic and diastolic function in elderly compared to younger. So the volume of treatment, especially in acute myocardial infarction, in spite of gerontological principles of "small intervention” should not be smaller than in the young, since age is an aggravating moment flow of MI and chronic heart failure (CHF).
