Hypertensive heart disease (HD) is the most common cardiovascular disease among the working-age population, causing the highest percentage of mortality and disability. Even in Western European countries, the achievement of the target level of blood pressure (BP) against the background of the use of modern antihypertensive therapy does not exceed 35%. This is most likely due to the mosaic nature of the pathogenesis of hypertension and the underestimation of the significance of various mechanisms of regulation and increase in blood pressure. An important role in the development and progression of HD is assigned to endothelial dysfunction, which, in relation to the development of HD, is not only primary, but also secondary. This statement is also true for neuro-humoral changes in hypertension, in particular, hypersympathicotonia.
