The effect of pain syndrome and movement restriction on the psychoemotional state of patients with the initial stages of posttraumatic crusarthrosis

Purpose of the study: to evaluate the effect of pain syndrome and movement restriction on the psychoemotional state of patients with the initial stages of post-traumatic crusarthrosis.Materials and methods. The study was conducted on the basis of Center for Traumatology and Orthopedics in the period from 2013 to 2017. Out of 82 patients with pain syndrome and impaired ankle function, 2 study groups were formed 6 months after osteosynthesis after ankle fractures. The main group (39 patients) underwent removal of metal structures in combination with therapeutic and diagnostic arthroscopy, the control group (43 patients) was treatedconservatively. The relationship of pain syndrome according to VAS, limitations of ankle joint function according to AOFAS, features of psychological status according to the Beck, Spielberg scales before treatment, 1 year, 3 and 5 years after the start of the study was evaluated.Results. After 1 year of VAS, the pain level in the control group was 2.82points (p=0.042), in the main group - 2.14points (p=0.039). The indicators of functional status in the control group increased from 70.47to 86.31(p=0.049), in the main group from 72.34to 91.73 (p=0.028). After 3 years in the control group, according to the data, YOUR pain syndrome progressed and amounted to 5.63points, according to AOFAS, the functional result decreased to 61.27. According to Beck's questionnaire, mild depression was registered (13.54). By the end of the study, the AOFAS score was 44.12points, patients had moderate depression (19.68). No negative dynamics was registered in the main group, after 3 years (84.19) and after 5 years (82.86), patients had a good result on the AOFAS scale. There was no statistical difference. According to the Spielberg questionnaire, it was revealed that patients, after arthroscopy, had a low level of personal and situational anxiety during the entire observation period, there is no data for the presence of depression according to the Beck questionnaire. In the control group, situational anxiety increased to a moderate level after 5 years (37.19points)Conclusions. As a result of the study, it was found that pain syndrome and restriction of movements in the ankle joint 6 months after osteosynthesis of ankle fractures can cause depression and anxiety in 16.8% of patients, therapeutic and diagnostic arthroscopy of the ankle joint in combination with the removal of metal structures helps to reduce pain syndrome by more than 2 times (p=0.039), improve the functioning of the ankle joint from satisfactory to good condition (p=0.041) and reduce the frequency of depressive manifestations and anxiety 9 times(p function show_eabstract() { $('#eabstract1').hide(); $('#eabstract2').show(); $('#eabstract_expand').hide(); }
