Prevention of acute postoperative pancreatitis in surgical treatment of gastric cancer

The prospective study of the efficacy of octreotide-depo was carried out to prevent acute postoperative pancreatitis in surgical treatment of stage II-III gastric cancer. The study included 54 patients who underwent surgery with D2 lymphodissection. The study group patients (n=28) received octreotide-depo at a single dose of 20 mg given 7 days before surgery. No additional pancreatoprotective therapy was administered. The control group patients (n=26) received octreotide and protease inhibitors in standard doses during surgery and in postoperative period. The level of enzymaemia after surgery was significantly less in the study group than in the control group. Prophylactic use of long-acting form of ocreotide resulted in a statistically significant reduction in the frequency of postoperative complications pathogenetically related to surgical pancreatic injury, such as acute postoperative pancreatitis, reactive left-sided pleuritis and stable intestinal paresis
