Adaptive potential as a predictor of exacerbation risk in women with chronic pelvic pain during early rehabilitation

Aim. This study aimed to evaluate the role of adaptive potential as a predictor of exacerbation risk and a tool for personalizing early rehabilitation in women with chronic pelvic pain (CPP). Materials and methods. This simple randomized trial included 61 women with CPP and myofascial syndrome allocated to one of two groups. The experimental group (n = 33) received a rehabilitation program with monitoring of adaptive potential and autonomic nervous system status. The control group (n = 28) received a standard rehabilitation protocol. Primary outcomes included changes in adaptive potential, the Kerdo Index, and measures of central sensitization. Results. The experimental group demonstrated greater reductions in adaptive potential scores, Kerdo index values, and measures of central sensitization. An early allostatic increase in adaptive potential was observed at the beginning of rehabilitation in both groups; this response was less pronounced in the experimental group. Conclusion. The data obtained indicate that an elevated adaptive potential is associated with increased allostatic load, autonomic dysregulation, and sustained central sensitization. Thus, monitoring adaptive potential provides an effective tool for predicting exacerbation risk and individually tailoring rehabilitation programs in patients with CPP.
