Lipid peroxidation activity and state of antioxidative myocardium protection system during repair of congenital heart diseases in infants operated under extracorporeal circulation

The lipid peroxidation processes and the state of antioxidative protection system (AOPS) were studied in 40 infants suffering from congenital heart disease (CHD) and operated under extracorporeal circulation (ECC), while using crystalloid chemical/hyporthermic cardioplegia (CCHC) (11 patients), hemic cardioplegia (14 patients) and «Custodiol» solution (15 patients). An integrated assessment of the intraoperative content of free fatty acids (FFA), malonic dialdehyde (MDD), hepatocuprein (HC), cholesterol, catalase activity in arterial blood and coronary sinus samples was carried out. It was found out that activation of lipid peroxidation (LPO) and tension of the antiperoxy protection system took place during various stages of CHD repair. The dynamics of the content of LPO products and AOPS enzymes in arterial and coronary sinus blood was of a unidirectional nature and featured an increase in the concentration of MDD, catalase activity, a decrease in the level of HC and hypocholesteremia. More expressed hypocholesteremia was observed in patients with cyanotic heart diseases at a reperfusion stage. Postischemic and reperfusion disturbances of PLO in the myocardium are the factors of cardiocyte damage and contribute to pathogenesis of postoperative cardiac insufficiency development.
