Treatment algorithm poststenting scarring stenosis of the larynx and trachea

Were analyzed results of treatment of 98 patients with postintubational cicatrical stenosis of larynx and trachea преимущественно with II-III degree of narrowing. Localization: голосовые складки - 7, vocal folds and subglottic space - 4, subglottic space - 6, cervical part of trachea - 73, thoracic part of trachea - 8. In a case of cicatrical stenosis at the level of vocal folds we think that performing of laryngolysis and laryngeal bougienage by esophageal bougies with increasing diameter; in a case of cicatrical stenosis of subglottic space - laryngolysis with following bougienage and T-shaped silicone implant placement or laryngoplastics; in a case of asymmetric cicatrical stenosis of cervical part of trachea - mechanical or electrosurgical scar removing and bougienage by esophageal bougies or by bronchoscope cone; in a case of circular cicatrical stenosis of trachea - electrodestruction of more rigid parts of cicatrical circus, bougienage by esophageal bougies or by bronchoscope cone and endoprothesis replacement by different constructions of stents, tracheal resection.
