Артериальная гипертензия у реципиентов почечного трансплантата: патофизиология, диагностика, лечение

Актуальность. Артериальная гипертензия у реципиентов почечного трансплантата - основной фактор риска сердечнососудистых заболеваний, утраты трансплантата и преждевременной смерти. В посттрансплантационном периоде в 80 % случаев развивается стойкая или рефрактерная артериальная гипертензия, трудно коррегируемая привычной медикаментозной терапией. Трудности лечения заключаются в неэффективности многих препаратов первой линии, а также в том, что наиболее распространённые иммуносупрессивные препараты (циклоспорин, такролимус и метилпреднизолон) способствуют развитию гипертонии. Данный вид артериальной гипертензии представляет собой значительную проблему в клинической практике из-за сложности лечения и высокой летальности. Необходимость изучения терапии посттрансплантационной гипертензии обусловлена не только клинической значимостью, но и потенциальной возможностью улучшения результатов лечения и продолжительности жизни реципиентов почечного трансплантата.Цель: изучение генеза, факторов риска, патофизиологии, диагностики и лечения посттрансплантационной артериальной гипертензии.Материалы и методы: проведён анализ отечественной и зарубежной литературы с целью изучения посттрансплантационной артериальной гипертензии. Анализировано 37 литературных источников.
