Incorrectly fused metacarpal head fractures are a serious problem in modern hand surgery. Dysfunction, persistent arthralgias and subsequent deformities cause a lot of problems for both the patient and the doctor. The presence of intra-articular fractures and free bone fragments leads to dysfunction of the metacarpophalangeal joints and, accordingly, to a decrease in the quality of life. Given the high level of development of traumatology and orthopedics in Russia, specialists are increasingly using arthroscopic surgery. It is one of the most promising in modern surgical practice. With its help, it is possible to diagnose damage not only to the capsular-ligamentous apparatus, but also to the articular cartilage. The use of this method made it possible to increase theaccuracy of the diagnosis, get a complete picture of the nature of the injuries and substantiate the tactics of future surgical treatment. Low invasiveness and low complication rate significantly reduce the patient's stay in the hospital and accelerate the rehabilitation process. However, when choosing a treatment, it is necessary to take into account complaints, clinical manifestations and data from laboratory and instrumental examination methods. In the given clinical case, we chose a minimally invasive arthroscopic technique for treating an incorrectly fused comminuted fracture of the second metatarsal head with intra-articular fragments.
