The article presents a clinical case of chronic radiation dermatitis that occurred five years after the end of radiation therapy for breast cancer. The clinical picture of lesions was localized in the projection of exposure to ionizing radiation. The rashes were represented by erythematous areas, foci of atrophy, multiple telangiectasias, cracks, peeling, and a single ulcer. Dermoscopy revealed areas of whitish and waxy skin atrophy, telangiectasias, hyperpigmentation, peeling, a single ulcerative defect, covered with crusts. The duration of the period from the moment of exposure to the etiological factor until the onset of skin lesions makes it difficult to diagnose this disease. Timely diagnosis is possible when taking into account the totality of anamnestic and objective data and knowledge of the clinical features of radiation damage to the skin.
