We have studied the concentration of interferon (IFN) and interleukin18 (IL18) by enzymelinked assay in the serum of patients with acute respiratory viral infections (ARVI) in acute and convalescence periods of the disease. A total of 85 patients aged 15-54 years old were studied: influenza pH1N1 (n=18), influenza A/H3N2 (n=17), adenovirus infection (n = 17), parainfluenza (n=20) and respiratory syncytial virus infection (n=13). A separate group (n=10) consisted of fatal cases of patients with severe influenza pH1N1 at the age of 19-61 years. The blood of healthy donors (n=17) was a control point. A significant increase in IFNγ in the acute stage of influenza pH1N1 (especially when it is severe), influenza A/H3N2, adenovirus infection, parainfluenza and respiratory syncytial virus infection (compared with control) do not normalize by the period of convalescence what confirms its importance in the pathogenesis of the disease. The concentration of IFNα was elevated in the acute stage of influenza pH1N1, parainfluenza, respiratory syncytial virus infection with the normalization by the period of convalescence. We have defined an increase in IL18 in patients with severe influenza pH1N1. The concentration of IL18 in other groups of patients were within normal limits. Conclusion: The prognostic significance of high IFNγ and IL18 stimulator in severe influenza pH1N1, which was used to create an expert system "Expressdiagnosis and prognosis of ARVI" was established.
