Effects of KRT device in the treatment of congested heart failure

The research consisted of 92 patients with heart failure of III-IV functional class (NYHA) and left branch of bundle His block which were divided into two groups. The first group consisted of patients (n=39) who underwent a study of KRT device influence on systolic and diastolic myocardial function with the help of radionuclide methods. Patients of the second group (n=53) underwent analyses of ventricular arrhythmia course. According to the data received from radionuclide research methods: the systolic myocardial function improved due to percentage increase of the circular fibres reduction, and diastolic function improved due to filling fraction increase during 1/3 and 2/3 diastoles. Because of KRT 10 patients with earlier recorded ventricular extrasystole displayed progress of ventricular rhythm disturbances: 5 patients got volley ventricular extrasystole and ventricular tachycardias were registered in 5 patients. Reasoning from received data, one can conclude that patients with expressed clinical presentations of heart failure and ventricular dyssynchrony need to undergo transplantation of biventricular cardiostimulators, combined with the function of defibrillator to initially prevent sudden death. Further research is required to define if arrhythmogenic effects conditioned by KRT device or progress of pathologic process in ventricular myocardium disappear or reduce.
