Экстирпация пищевода при кардиоспазме IV стадии и его рецидиве после открытой и эндоскопической кардиомиотомии

Широкое применение в клинической практике лапароскопической кардиомиотомии с различными модификациями полной или неполной фундопликации, часто выполняемой в качестве первичного и единственного способа лечения без учета стадии болезни, неизбежно приводит к появлению пациентов с рецидивом дисфагии, развитием пептического рефлюкс-эзофагита и прогрессированием заболевании. Трансхиатальная экстирпация пищевода с одномоментной заднемедиастинальной эзофагопластикой изоперистальтической желудочной трубкой и пищеводным анастомозом на шее является операцией выбора у больных с IV стадией кардиоспазма (ахалазии кардии). Целесообразность выполнения повторной операции на кардии после неудачного первичного вмешательства весьма сомнительна.
