В обзоре освящены процессы патогенеза и клиники новой коронавирусной инфекции. Согласно результатам исследований, изучавших патогенез SARS-CoV и MERS-CoV, тромботические осложнения являются наиболее характерными для инфекций, вызванных коронавирусами, и инфекция SARS-CoV-2 не является исключением. Полиорганное поражение вызвано наличием нескольких факторов: вирус тропен к тканям, богатым ферментом АПФ2, экспрессирующемся в таких важных структурах, как желудочно-кишечный тракт, почки, слизистые оболочки дыхательных путей, сердечно-сосудистая и нервная система. Во-вторых, активация системы комплемента и нарушение урокиназного пути объясняют тромбозы. Влияние эстрогена на защиту эндотелия от окислительного стресса объясняет более высокую смертность у мужчин. Таким образом, подробное изучение патогенеза может повлиять на терапевтический подход к лечению данного заболевания и способствует предупреждению тяжелых осложнений.
