Энурез - теоретические основы и практические рекомендации

Ночное недержание мочи (энурез) достаточно часто встречающееся заболевание у детей, его лечением занимаются не только детские урологи, но и нефрологи, педиатры, неврологи. Согласно определению Международного общества по проблемам недержания мочи у детей (ICCS) энурезом считается непроизвольное интермиттирующее мочеиспускание во время сна у ребенка старше 5 лет. Это состояние встречается у 10% детей в возрасте до 7 лет и является причиной психических и эмоциональных трудностей у детей и членов их семей. Распространенность недержания мочи уменьшается с возрастом. У детей в возрасте 4,5 лет ночное недержание мочи встречается в 21% случаев, тогда как в возрасте 9 лет - только у 8% детей. В настоящее время считается, что в патогенезе энуреза основными факторами являются ночная полиурия, ночная гиперактивность детрузора и высокий порог пробуждения. Возможно, в основе всех этих проявлений лежат нарушения на уровне центральной нервной системы. Препаратом первой линии в лечении заболевания, в случае отсутствия сопутствующих заболеваний, которые могут быть его причиной (сахарный диабет, заболевания почек, аномалии мочеполовой системы) является десмопрессин. При наличии сопутствующих нервно-психических расстройств у детей с энурезом оправдано назначения комплексной терапии с применением Пантокальцина. Также применяются холинолитики, антидепрессанты, поведенческая и аларм-терапия.
