Summary We describe an original method of estimation of testicular parenchyma echogenicity during ultrasound examination. The developed program is designed for the analysis of digital graphic images "Eye-plus". 63 boys aged 2 - 14 years (6,7±3,2) treated for "acute scrotal syndrome" were examined. The proposed algorithm for the analysis of digital ultrasound images and the computer program that implements it allow to obtain additional parameters characterizing the changes in the testicular parenchyma in acute diseases. This contributes to the objectivity of assessment of a number of ultrasonic parameters in the state of gonads in acute testicular diseases and reduces the degree of operator-dependency of ultrasound examination. The results can be used to determine surgical tactics. Patients with reduced blood flow should be operated urgently, because it indicates the presence of acute testicular ischemia, which is a critical condition, regardless of whether it is due to the twist of the spermatic cord or external compression due to tissue edema. Objectively recorded increase in blood flow indicates the presence of inflammation, which reduces the urgency of the intervention and leaves time for further examination and conservative therapy.
