We present a clinical case of rare metastasis of peripheral cancer of the upper lobe of the right lung into the wall of the cecum in a 66-year-old patient after complex treatment (surgical - upper lobectomy on the right with mediastinal lymphadenectomy + chemotherapy + radiation therapy). Metastasis manifested itself with symptomatic abdominal pain 11 months after completion of treatment. A comprehensive examination (ultrasound of the abdominal cavity and colonoscopy) revealed a tumor lesion of the cecum dome and the baugin valve. The patient underwent neoadjuvant chemotherapy, and then laparoscopic right-sided hemicolectomy with side-to-side ileotransverse anastomosis. Histological and IHC examination of the surgical material revealed thatthe tumor corresponds to metastasis of low-grade lung adenocarcinoma. Due to the timely treatment of the patient and the diagnosis, it was possible to carry out treatment before the development of severe complications - intestinal obstruction, intestinal bleeding, perforation of the intestinal wall and peritonitis. Currently, the patient's condition is satisfactory, he leads an active lifestyle, receives immunotherapy with pembrolizumab.
