Overcoming treatment resistance in patients with schizophrenia is one of the urgent tasks at the stages of the modern psychiatry. Use of modern methods of instrumental diagnosis (PET with 18F-FDG) showed morphological differences between hallucinatory and delusional variants of paranoid schizophrenia. After the treatment one patient with hallucinatory variant of paranoid schizophrenia had complete reduction of hallucinatory symptoms, which was reflected in PET by a partial restoration of glucose metabolism in the frontal and temporal lobes of the brain. The observed changes allow us to speak about the heterogeneity of the neurophysiologic mechanisms leading to resistance, even in a clinically homogeneous group, which leads to the idea of an individual approach to therapy.
