Reconstructive surgery in patients with postoperative ventral hernia

The aim of the study was to assess surgery results of patients with postoperative ventral hernia with using polypropylene prosthesis (mesh) for reconstruction of frontal abdomen. 184 patients were divided in three groups depending on a method of surgery: onlay - 38 patients, inlay - 33, sublay - 113. Complications were in 26 cases (14.1%): seromas - 11.4%, hematomas - 1.0%, skin necrosises - 0.5% and infectious complications - 1.0%. In group 1 - 9 (23.6%), in group 2 - 8 (24.2%), in group 3 - 9 (7.9%) cases of complications. Disease relapses comprised 4.7% in group 1, 12.5% in the second and at 7.6% in group 3. Reconstructive surgery of hernia defects using synthetic prosthesis allowed significantly lowering the amount of relapses of hernia. The sublay location of limb, absence of contact with subcutaneous tissue allowed preventing postoperative fluid accumulation. The relapses of hernias after reconstructive surgery of frontal abdomen developed within the first year after surgery.
