Estimated results of radical cystectomy

Objective. The objective of the study is to estimate the results of radical cystectomy in patients with bladder cancer. Materials and Methods. A total of 96 patients aged between 31 and 74 years were enrolled in the trial. All the patients had bladder cancer and underwent radical cystectomy (Studer technique and Bricker procedure) at Lipetsk Regional Oncology Centre in the period from 2005 to 2016. All the patients underwent clinical laboratory, X-ray, endoscopic, and histological examination as well as MRI and ultrasound examination. Results. Early postoperative complications were observed in 42 patients (43,8 %). Postoperatively, ureteral-intestinal anastomotic leak was detected in 4 patients (4,2 %) who underwent relaparotomy, sanitation, and abdominal drain procedure. Acute postoperative pyelonephritis was noted in 10 patients (10,4 %). That may be attributed to the technical ureteral-intestinal anastomosis imperfection (end-to-side anastomosis), which may cause gastro-ureteral reflux Besides, 16 patients (16,7 %) demonstrated dynamic bowel obstruction, and 12 patients (12,5 %) had wound inflammatory complications. Postoperative lethality was 4,2 %. Conclusion. The techniques of ureteral-intestinal anastomosis during radical cystectomy and conservative treatment in the early postoperative period should be improved to reduce complications in patients with bladder cancer.
