Features of affective symptoms in the structure of non-psychotic mental disorders of organic origin

Background. Organic mental disorders are characterized by polymorphism of symptoms, presence of complex etiological and pathogenetic relationships, a tendency to a protracted progressive course and are difficult to treat. Objective: to study the relationship of affective symptoms with the course of non-psychotic mental disorders of organic origin and the sustainability of treatment results. Materials and Methods. In 2014-2020, 165 patients with organic non-psychotic mental disorders aged from 30 to 65 years were examined using clinical-psychopathological, clinical-dynamic, clinical-follow-up, psychological, statistical methods. After using the exclusion and dropout criteria at the follow-up stage, the main group consisted of 64 patients with organic non-psychotic mental disorders and comorbid affective symptoms at the stage of long-term effects of therapy, the comparison group consisted of 50 patients with adjustment disorder. Results. Analysis of the nosological structure showed the predominance of organic emotionally labile disorders, organic personality disorders were less common. All patients underwent examination and courses of individually developed therapeutic and prophylactic measures implemented in the form of differentiated rehabilitation programs. A fairly high initial efficacy of the treatment was observed. After discharge from the hospital of patients with organic non-psychotic mental disorders, frequent addition of previously uncommon affective symptoms was noted, which was one of the circumstances that influenced the long-term results of treatment. In 2020, a higher incidence of depressive manifestations was recorded in comparison with the previous period of observation of such patients, which we explain by situational circumstances (the influence of pandemic and coronavirus stress and restrictive measures). The article also discusses the issues of diagnosis, clinical picture and therapy in terms of overcoming the negative health consequences of patients with organic mental illness and improving the quality of treatment. Thus, the correction of secondary affective symptoms and a more adequate consideration of socio-psychological circumstances make it possible to increase the effectiveness of treatment and rehabilitation of patients in the organic register
