Adequate selection of patients for different revascularization approaches is the key in achievement of successful outcomes of the procedures. The main part of the intraoperative and postoperative risk stratification is an evaluation of angiographic coronary artery disease (CAD) severity. The use of currently available indexes does not allow rigorous prediction of risk for percutaneous coronary intervention (PCI). One of the approaches to increase predictive value of integrated clinical and angiographic scores is their combination with molecular genetic data. According to the recent studies, small locus of 9 chromosome (9p21.3) is significantly associated with the risk of myocardial infarction, CAD outcome, severity and progression rate. The present review discusses recent advances in the novel promising area of research aimed at the use of genomic data in risk stratification of patients before PCI to accurately predict coronary intervention outcomes.
