Приверженность лечению кардиологических больных: подходы к оценке, пути повышения и прогностическое значение

Под приверженностью лечению понимают комплексную модель поведения пациента в отношении своего здоровья, реализующуюся в степени соответствия такого поведения относительно рекомендаций, полученных от врача в отношении приема препаратов, алгоритмов самоконтроля, соблюдения диеты и других мер изменения образа жизни. Оценивать приверженность можно, определяя в биологических жидкостях организма (кровь, моча) метаболиты лекарственных препаратов, методом прямого контроля приема препаратов или подсчета пустых блистеров и количества выкупленных рецептов, а также с помощью многочисленных опросников и анкет. При этом важная роль в повышении приверженности терапии отводится лечащему врачу, без контакта с которым большая часть больных принимает самостоятельное решение о прекращении приема всех или отдельных назначенных препаратов или необоснованной коррекции их дозировок. К факторам, оказывающим влияние на снижение приверженности лечению кардиологических больных, относятся: неправильное понимание пациентом собственного заболевания и ожидаемых эффектов терапии, страх перед нежелательными эффектами терапии, низкий уровень мотивации, склонность к забывчивости и некоторые другие. Неприверженности способствует также полиморбидность и связанная с ней полипрагмазия, особенно в случае назначения препаратов разными специалистами - терапевтами, эндокринологами, урологами, неврологами, офтальмологами. При это отмечается явный рост нежелательных последствий (повторные госпитализации в связи с инфарктом миокарда, сердечно-сосудистыми событиями) у неприверженных больных спустя уже 6 месяцев после предыдущего коронарного события и существенно более высокий риск спустя год. Повышение приверженности лечению - задача медицинских работников, начиная со стационарного этапа лечения (четкие рекомендации, отмеченные в выписных документах), с последующим сопровождением врачей амбулаторного звена, объяснением необходимости приема тех или иных медикаментов, и заканчивая контролем за исполнением врачебных назначений.
