The political elite of Eastern Siberia was formed from appointees, natives of European Russia, who received an education and began their careers in the center of the empire. Before the eyes of any capital appointee to the governor's post in Eastern Siberia, regardless of the time, there was a prospect of immersion in a large volume of specific problems. Medical problems retained their constancy in the deepest level of backwardness. In the paradigm of the rational approach, we will consider the initiatives of regional officials in the field of public health, and determine the motivational attitudes of government representatives aimed at medical and social changes. The authors put forward the idea that representatives of the regional authorities considered public health from the position of a rational approach, and personal qualities acted only as a supplement to managerial abilities. The authors of the publication believe that a large number of works have been accumulated covering the activities of representatives of regional authorities in the East Siberian region. However, there is a gap in the study of the factors that prompted representatives of the local political elite to carry out medical and social transformations, as well as to implement measures to increase the network of hospitals, resolve personnel issues, and to change the system of medical care to the population.
