Риски лучевой терапии для детей и подростков. Обзор литературы

Начиная с 50-х годов 20 века, лучевая терапия активно применяется при лечении опухолей у детей. По мере накопления клинических наблюдений стало ясно, что при высокой непосредственной эффективности метода его отдаленные результаты могут нивелироваться за счет развития отдаленных последствий. Как выяснилось, вероятность развития тяжелых и даже инвалидизирующих последствий зависит от возраста ребенка в момент лечения, величин разовых и суммарных доз, объемов облучения. С развитием технических средств лучевой терапии, созданием новых противоопухолевых препаратов стало возможным в ряде ситуаций отказаться от применения лучевой терапии, уменьшать дозы и объемы облучения, однако полностью исключить радиационное воздействие на критические структуры удается далеко не всегда. Поэтому представилось целесообразным провести анализ имеющейся в литературе информации о возможных последствиях радиационного воздействия на различные органы и системы у детей. Знание этих рисков должно позволить врачам, осуществляющим лечение и динамический мониторинг за детьми, получавшими лучевую терапию, предупреждать и своевременно выявлять развитие возможных радиационных повреждений.
