Scintigraphic evaluation of structural and functional changes of myocardium and alveolar-capillary permeability of lungs in patients with community-acquired pneumonia

Aim: to evaluate the myocardium state of patients with community-acquired pneumonia by radionuclide methods. Our study involved 39 patients (middle age 25.7±1.4) with community-acquired pneumonia. On 2-5 day of hospitalization 12 patients underwent Gated-SPECT with 99mTc-tetrofosmin, 13 patients underwent SPECT with 99mTc-pyrophosfate, 14 patients underwent ventilation lung scintigraphy. Gated-SPECT with 99mTc-tetrofosmin showed that 9 of 12 patients had moderate perfusion abnormalities (middle size of perfusion defect was 4.6±0.7%). Disturbances of local contractility evaluated by summered motion score (SMS 3.6±0.6) were marked in 10 patients and in 8 patients they evaluated by summered thickening score (STS 5.2 ± 1.0). The pathological uptake of 99mTc-pyrophosfate was marked in 70% of examined people. All patients with community acquired pneumonia show increased lung permeability rate. Thereby, the majority of patients with outof&hospital pneumonia had scintigraphic signs of myocardium and lung tissue injury.
