Contemporary approaches to outpatient care management for cardiovascular disease prevention

The lack of efficacy of outpatient prevention measures results in the need to develop and implement new methodological approaches in the primary care system with an obligatory use of medical information systems and the creation of an integrated registry for patients at high risk of cardiovascular diseases (CVD). A new approach to CVD prevention is based on the calculation of total cardiovascular risk using the proprietary medical information system. Between 2010 and 2012 the Registry included 4508 patients, of those 1926 (42.7%) did not have any CVD and 2582 (57.3%) patients did. These patients were included in the electronic database called ‘Registry for risk stratification and CVD prevention’. The preventive technology was assessed based on the results of the efficacy of preventive medical check-up in arterial hypertension patients (62 patients), who were subjected to the new approach of preventive medical care, as well as on the general efficacy of CVD prevention in an outpatient clinic. The study results showed the value of the proposed prevention technology for clinical practice: the strategy could enhance the availability of individual preventive recommendations, starting from pre-hospital care, and allows to form the follow-up groups who will be assessed for cardiovascular risk.
