Comparative study of the possibilities of SPECT/CT and magnetic resonance imaging whole body in the diagnosis of bone metastases

Purpose. Comparative assessment of the possibilities of SPECT / CT and whole-body MRI in the detection of metastases of the axial skeleton. Materials and methods. The study included 22 patients with malignant tumors of various locations and suspected secondary bone lesions. All patients underwent SPECT / CT on the device Philips "Precedence" using mono- and diphosphates of Tc-99m activity of 370-600 MBq, as well as MRI technique "whole body" on the magnetic resonance imaging "Atlas" 1.5T. The diagnosis of secondary bone disease was later confirmed by further observation for at least 6 months. Results. Metastatic bone lesions were present in 18 (81.8%) patients, 4 (18.2%) patients the changes were not metastatic nature. As a result of the use of MRI techniques "whole body" was made 1 false-negative and false-positive diagnosis 1, the sensitivity was 94.4% (17/18), specificity of 75% (6/8). According SPECT / CT was observed as the presence of 1 false-negative and false-positive 1, sensitivity was similar techniques 94.4%, specificity - 75%. The accuracy of both methods was 92.3% (24/26). Conclusions. In the diagnosis of bone metastases SPECT / CT and MRI "whole body" are complementary methods by different principles of image acquisition. Both methods have their advantages and disadvantages that must be considered in the appointment and evaluation of the results of the research.
