Tactical/technical aspects of complex treatmentof purulent pericarditis with compression heart syndrome and multiple organ failure

Eighteen patients with purulent pericarditis were treated, their mean age being 42± 16.6 years.The severity of multiple organ dysfunction syndrome score (SOFA) at admission was 2.03±0.3. Cardiac tamponade was observed in all cases. Complex treatment consisted of antibiotic and anti-inflammatory therapy, pericardiocentesis and subsequent pericardiotomy, pericardioscopy and endovideoassisted debridement of the pericardium. The severity of multiple organ failure decreased by 1.8 times by the 3rd postoperative day and was equal to 0.5±0.1 by the 7th postoperative day. Lethality, disease recurrence or pericardial constrictions were not observed.
