The effect of hypoglycemic therapy on kidney function in patients with diabetic nephropathy

Type 2 diabetes mellitus (T2DM) is a global health problem worldwide. This is due to the high prevalence of the disease and its complications among the working population. One of the significant microvascular complications of diabetes mellitus is diabetic nephropathy (DN), which develops in 40-60% of type 2 diabetes. To study the effect of hypoglycemic therapy on renal function in patients with diabetic nephropathy. The study included 120 patients with type 2 diabetes, whose average age was 60.1±7.6 years. Inclusion criteria were: men and women aged 35 to 65 years with type 2 diabetes with a diabetes duration of up to 10 years and an eGFRcr of at least 60 ml/min/1.7 m2. The results of our study showed that in patients with DN 1 (C1,A1) with normoalbuminuria, the drugs iDPP-4 and iSGLT-2 had a unidirectional positive effect on the main indicators of the functional and structural state of the kidneys. Thus, by the end of the 6th month of observation, both in the group of patients taking iDPP-4 as part of hypoglycemic therapy and in the group of patients taking iSGLT-2, the creatinine level and estimated GFRcr on average were comparable with the initial level (92.20±4.41 versus 93 .31±4.61 ml/min/1.73m2- in group DN1+S and 92.5±5.41 versus 93.12±5.67 ml/min/1.73m2-in group DN1+E). The drugs iDPP-4 and iSGLT-2 have a unidirectional, comparable effect on the main indicators of carbohydrate metabolism and the achievement of target HbA1c levels in patients with type 2 diabetes, regardless of the severity of diabetic nephropathy.
