Возможности и ограничения спекл-трекинг стресс-эхокардиографии

Стресс-эхокардиография является методикой, которая позволяет оценивать сократимость миокарда и внутрисердечную гемодинамику в условиях физической нагрузки или реакции на фармакологический препарат. Оценка систолической функции левого желудочка (ЛЖ) играет важную роль в установлении прогноза и выборе стратегии лечения при различных сердечно-сосудистых заболеваниях. Технология спекл-трекинг - одна из наиболее перспективных для определения значений деформации ЛЖ, благодаря которой возможна не только визуальная, но и количественная оценка сократимости миокарда ЛЖ как в состоянии покоя, так и при проведении нагрузочных тестов. В данном обзоре проанализированы возможности применения спекл-трекинг стресс-эхокардиографии c физической нагрузкой или с введением фармакологического препарата у здоровых лиц и пациентов с различными заболеваниями, такими как ишемическая болезнь сердца (ИБС), клапанные пороки сердца, сахарный диабет, микрососудистое поражение. Наряду с этим рассмотрены различия между значениями продольной систолической деформации ЛЖ в зависимости от возраста, а также проведено сопоставление данных спекл-трекинг стресс-эхокардиографии с результатами коронарной ангиографии и компьютерной томографии коронарных артерий.
