Objective: to demonstrate a rare clinical case of synchronous primary multiple metastatic cancer, to identify the features of the diagnostic picture and to provide a brief review of the literature. Materials and methods. Patient B., born in 1972, complained of headache and periods of dizziness. Initially, he was directed to magnetic resonance imaging of the brain, followed by the conclusion of multiple focal lesions of both hemispheres. He was examined using multispiral computed tomography, fibrobronchoscopy, esophagogastroscopy and pathomorphological examination. Results. The examination revealed the synchronous development of neuroendocrine small cell lung cancer and adenocarcinoma of the cardioesophageal junction, confirmed by morphological examination. According to the MRI data, multiple brain formations suspected of secondary changes were visualized. According to MCT data, a bulky formation in the upper mediastinum in a conglomerate with lymph nodes and a circular thickening of the lower third of the esophagus with a transition to the subcardial part of the stomach were visualized. Conclusion. The presented clinical case demonstrates a wide variability in the differentiation of primary multiple malignant neoplasms and the specificity of the course using the example of our own observation.
