Diagnostic of mielopathies of venous origin and integrated treatment

Objective: Exploring the characteristic clinical and diagnostic aspects of the myelopathies of venous origin on the basis of pathogenetic mechanisms and to develop the principles of integrated treatment according to anatomical and topographic studies in correlation with selective phlebography. Material and methods: examined 62 patients with unknown etiology spastic para - and tetraparesis. Algorithm research - MRI of the spinal complex and selective phlebography. Main phlebographic signs - atresia, compression, stenosis. There were performed 32 interventions on 28 patients. Results: satisfactory results were obtained in 22 cases (78.6 %). Conclusions: The violation of blood flow on major tributaries of caval veins systems in the spinal canal develops venous hypertension. The causes of such violations are located outside the spinal canal in the "bottlenecks places" and verified by selective phlebography. The main clinical symptom - spastic movement disorders. Surgical treatment should be directed to eliminate those causes and restore a physiological venous blood flow.
