Dynamics of cellular immunity parameters during autologous hemopoetic stem cell translation in patients with multiple myeloma

The purpose of the study was acomprehensive assessment of the cellular immunityin patients with multiple myeloma (MM) beforeand after autoHSCT. The study included 123patients with MM who underwent autoHSCT inthe Center of Bone Marrow Transplantation ofthe Russian Research Institute of hematologyand transfusiology for the period from 2014 to2019. All patients received bortezomib-containingtherapy using scheme CVD (Cyclophosphamide +Bortezomib + Dexamethasone) or VD (Bortezomib +Dexamethasone). Most often hemopoietic stem cellmobilization from peripheral blood was performedusing Vinorelbine (single dose 35 mg / m2, bolusintravenously) or Cyclophosphamide (singledose 3-4 gI m2) with subseguent G-CSF injection(Lenograstim or Granocyte) - 10 |ig / daily,subcutaneously in two equal doses. Conditioningregimen (high-dose chemotherapy) prior toautoHSCT included infusion of melphalan fortwo days, total dose 200 mg I m2. In patients withsevere comorbidity total melphalan dose wasdecreased to 140 mg I m2. Using multi-parameterflow cytometry and monoclonal antibodies anti-CD3, CD4, CD8, CD16+56, CD25 and CD19 thepercentage of lymphocyte subsets of peripheralblood was assessed before autoHSCT (at the startof conditioning, after autoHSCT - at the day ofthe end of leukopenia (WBC > lxlO9/L, on averageat 14th-30th day), at D+100, D+180 and D+360.It was found that prior to the auto-HSCT at thestart of the conditioning regimen, the percentageof mature T-lymphocytes (CD3+) and the mainimmunoregulatory populations (CD3+CD4 + andCD3+CD8+) were within normal limits and did notdiffer from those in the control group.
