Фибрилляция предсердий у пациентов с ишемической болезнью сердца: современное состояние проблемы

Фибрилляция предсердий - одно из самых распространенных нарушений ритма сердца, наблюдаемых в клинической практике. Несмотря на успехи в диагностике и лечении, фибрилляция предсердий остается одной из ведущих причин сердечно-сосудистой летальности и заболеваемости. Кроме того, фибрилляция предсердий достаточно часто сочетается с другой патологией сердечно-сосудистой системы и является маркером неблагоприятного исхода. Несколько проведенных ранее исследований продемонстрировали снижение выживаемости пациентов с ишемической болезнью сердца и фибрилляцией предсердий, у которых не проводилось хирургическое лечение аритмии. Согласно другим данным, наличие предоперационной фибрилляции предсердий среди пациентов, перенесших изолированную операцию аортокоронарного шунтирования, было связано со значительно более высокими показателями основных послеоперационных осложнений. На сегодняшний день ни у кого не вызывает сомнения тот факт, что отсутствие хирургического лечения фибрилляции предсердий во время операции коронарного шунтирования является фактором риска повышенной госпитальной летальности, послеоперационной заболеваемости и приводит к снижению долгосрочной выживаемости. Проведенные исследования подтверждают необходимость хирургической абляции фибрилляции предсердий во время коронарной реваскуляризации с целью снижения как краткосрочной, так и отдаленной послеоперационной смертности и поздних осложнений.
